Two Drivers Killed in Head-On Crash on State Route 741 in Warren County, Ohio

Two drivers have died following a devastating head-on collision along State Route 741 in Turtlecreek Township, Warren County, Ohio, authorities confirmed.

The fatal crash occurred late Friday night and is currently under investigation by the Ohio State Highway Patrol, which is working to determine the exact cause of the collision.
According to investigators, the accident happened at approximately 10:00 p.m. on March 13, 2026, when two vehicles traveling in opposite directions collided head-on.
Authorities say a 2019 Tesla Model 3 traveling southbound reportedly crossed the center line and struck a 2020 GMC Terrain traveling northbound on the highway.
The crash occurred along a stretch of State Route 741 between Hamilton Road and State Route 63, an area frequently used by commuters traveling through Warren County.
The force of the collision caused catastrophic damage to both vehicles, leaving emergency responders with a severe crash scene when they arrived.
Emergency crews confirmed that both drivers died at the scene due to the severity of their injuries.
Authorities identified the victims as:
- Dylan Sougstad, 17, of Cincinnati, driver of the Tesla
- Chasity Smith, 53, of Trenton, driver of the GMC Terrain
- Officials reported that no additional passengers were inside either vehicle at the time of the crash.
Following the crash, first responders from local fire departments, emergency medical services, and law enforcement quickly secured the area.
A section of State Route 741 was closed for several hours to allow investigators to document the crash scene and remove debris from the roadway.
Traffic in the area was diverted while accident reconstruction specialists examined the wreckage.
The Ohio State Highway Patrol Lebanon Post is currently leading the investigation.
Authorities say they are examining several possible contributing factors, including:
- driver distraction
- excessive speed
- fatigue
- road conditions
- mechanical failure
Investigators have not yet determined why the Tesla crossed the center line.
